About the Program
The Case Management and Referral Program provides individualised support to community members by assessing their specific needs and connecting them with appropriate resources. This includes referrals to housing assistance, mental health care, employment services, legal aid, and other relevant supports. The program works closely with other departments and external partners to ensure a holistic, culturally sensitive, and accessible approach to care. Its goal is to empower participants to navigate complex systems and move toward greater stability and inclusion.
Our Program supports the community by:
Providing one-on-one guidance and personalised support
Assisting with forms, applications, and service navigation
Connecting individuals and families to resources such as housing, health care, language services, and employment programs
Offering multilingual support in English, Cantonese, and Mandarin to reduce barriers
Promoting independence, confidence, and a stronger sense of belonging through trusted referrals and compassionate care
